How big should it be?
Rule of thumb: printed width ≥ scanning distance ÷ 10. Pick the surface above and the studio sizes the code for you, then checks the printed module size against what a press can actually hold.
Questions people actually ask
Any survey link works: a Google Form, Typeform, SurveyMonkey, or a feedback page on your own site. Keep the survey short — a scan-to-survey works best when it takes under a minute to finish.
Receipts, packaging inserts, event badges, waiting rooms, and the bottom of invoices. The closer the code is to the experience you are asking about, the better the response rate.
Not from the code itself — a static QR is just your link, generated in your browser with nothing tracked by UseQR. Your survey tool's own analytics will show responses, and you can add UTM parameters to the link to see which placement they came from.
You can change the survey's questions freely. Changing the link itself means printing a new static code — or editing the form at the same address so the old link keeps working.
